If you are sending us a memorial donation, please include the following information:

• Who the donation is in memory of
• Name and address of the Next of Kin you would like us to notify
• Name and address of person who will receive the tax receipt

You may also designate your donation to go toward research. A portion of all donations received each year go to the Alzheimer Society National Research Program.
